The ED RN Case Manager participates in and facilitates the processes to provide all aspects of Patient Care Management Program. The Emergency Services RN Case Manager works closely with the entire team of Emergency Services providers through concurrent assessment and assistance in assuring the admission status and admission plan of care is appropriate to address the acute care needs of the patient.

Principal Accountabilities:

The WHC/Emergency Services RN Case Manager will also facilitate the appropriate transition to the most effective level of care when the patient needs further assistance without acute care hospitalization.

The WHC/Emergency Services RN Case Manager works closely with the entire Care Management Program to assist in assuring the admission status and admission plan of care is appropriate to address the acute care needs of the patient.

The WHC/Emergency Services RN Case Manager understands the operations and functions of Unit Based Nurse Case Managers, Discharge Planner/Social Workers, Chaplaincy, and Palliative Care services and provides effective transition and communication to assure these services are provided to patients with identified needs.

The WHC/Emergency Services RN Case Manager provides a collaborative approach to transitional care management, discharge planning and utilization review for all patients seen in these departments.

This process will include concurrent evaluation and discussion with the Practitioners and Care Team Members throughout the WHC/Emergency Department visit.

The WHC/Emergency Services RN Case Manager position requires competency and ability to assist in all aspects of our Care Management Program to include strong clinical and communication skills.

The WHC/Emergency Services RN Case Manager will maintain Licensure and competency for a Registered Nurse Case Manager.

They will maintain the competency and skills to assist in all aspects of Case Management and Discharge Planning when needed.

The responsibilities of this position occur in real time and require staffing 24/7 for efficient coordination and collaboration.

The WHC/Emergency Services RN Case Manager works directly with the Care Management Supervisors for day to day communication of operational needs and reports to the Care Management Director.

Qualifications

Education:

Bachelor's Degree required

BSN or equivalent experience in utilization review or discharge planning required

Master's Nursing, Case Management or Related Field Desired preferred

Experience:

Minimum 2 years experience as Hospital Nurse Case Manager - Required

Nursing experience in Emergency Services - Preferred

Licensures/Certifications:

CA Registered Nurse - State Licensure - Required

Certified Case Manager - CCM - Certification by CMSA or ACMA Preferred
